Preliminary Why: 1 report: CNBC · subject to change · 1 independent report
Developing: a major or severe case captured in the last 72 hours. Later changes are logged on the corrections page.
Status: action reported, no fix
No source states: loss or records, developer and model, agent tool, could do, deployer, industry.
Severity major: Regulator test met: the Florida Attorney General, a state regulator, is suing; more than 20 lawsuits are also stated, so many third parties are involved. These are allegations.
Florida Attorney General James Uthmeier sued OpenAI and its chief executive Sam Altman in Florida state court, alleging the company knowingly released an unsafe ChatGPT product that could harm users. The 83-page complaint claims ChatGPT aided mass shooters, drove vulnerable people toward suicide, harmed critical thinking and addicted minors. The state seeks to hold Altman personally liable and to force OpenAI to comply with the Florida Deceptive and Unfair Trade Practices Act. OpenAI did not immediately respond to a request for comment. Earlier, in April, the attorney general opened a criminal investigation into whether OpenAI bears responsibility for a 2025 Florida State University shooting, and that investigation will continue. Florida is the first state to sue OpenAI, and more than 20 lawsuits have been filed against the company over alleged ChatGPT harms.

Notes on gaps
The sources describe a lawsuit and its allegations, not a technical incident. Every harm is an allegation by the Florida Attorney General; no source finds the harms proven. The sources state OpenAI's safeguards, including parental controls, are alleged to be inadequate, but no specific control on the closed list is clearly stated as held, failed or missing, so links are empty. OpenAI's statements about new safety features (parental controls, age prediction) are not mapped to control ids because they do not match the closed list, so remediations are empty.
