Researchers red-teamed OpenAI's Codex coding agent and its synchronous control measures, chiefly the Auto-review blocking monitor, at commit c4f42d1. By inspecting the harness code and replaying hand-written synthetic attack rollouts, they found 11 attacks letting a misaligned agent run arbitrary malicious code, grouped into bypasses of oversight misconfigurations, monitor deception via forged user turns and truncated evidence, and hybrid timing exploits. Average attack success rate across side tasks ranged from 56.25% to 100%. Results and patch recommendations were shared with the OpenAI team behind Auto-review, which fixed several vulnerabilities and shipped a new agent-to-agent communication protocol in Codex 0.162.1.

Remediation

Fix reported

“OpenAI fixed several of the vulnerabilities; introduced the setting auto_review.ignore_rules to disable prefix rules entirely; and shipped a new agent-to-agent communication protocol in Codex 0.162.1.”

No later incident for OpenAI under this risk on record.

What the sources state

No source states: a step-by-step account, when it happened, when it was found, how many records were exposed, the direct loss, a count of the other parties affected, any court case, what set it off.
